Youth Ministry Waivers

The Office of Youth Ministry is happy to announce that new youth ministry waivers will be available for use beginning January 1, 2018. The new forms will reduce the amount of paperwork for parents, chaperones, and youth ministers and seek to provide a comprehensive and easy-to-use annual medical and individual event format.

At the beginning of each school year parents should fill out an “Annual Emergency Medical Care Form”  and the "Electronic Communication Notification and Release / Photo and Media Consent and Release" Each youth ministry should store the “Annual Emergency Medical Care Form” in a locked file cabinet. Whenever a student participates in a youth ministry field trip or event, the youth minister must take this form from the student’s annual file and bring along with the individual event form. At the conclusion of the event, both forms should be placed in the students annual file located in a locked filing cabinet.


The form for individual events “Field Trip Permission and Release” is much simpler than the previous form, because it simply asks for the basic details of the field trip and asks the parent to sign a release for the Diocese of Fort Wayne South Bend, your parish, and anyone involved in the field trip. Medical information is no longer needed on the “Field Trip Permission and Release” because this need is filled by the presence of the “Annual Emergency Medical Care Form”


If you have volunteer drivers, be sure they fill out the “Driver Information Sheet”

Volunteers should sign a volunteer application before beginning their ministry. This is part of completing safe environment


High School age volunteers should also fill out a volunteer application.
